Phone Consultation
Before you make your first appointment, a free 15-minute phone consultation is required to ensure I understand your needs and can provide the best service possible. I look forward to speaking with you!
Consent Forms
The consent forms that are required before your first session are available below. I will discuss with you during the phone consultation which forms you will need to submit. These forms are digital and will be received immediately upon submission.
Most clients will only need to submit the following Consent Form. In some instances a medical or therapist referral may be needed in specific situations and will be determined during the 15 minute phone consultation. If it is determined that it is needed, the referral form will be sent to you through email. These forms will need to be printed to take to your doctor or therapist and require you to take a photo or scan once it is completed and sent back through email. (The Consent Forms that are required by everyone, listed below, are digital and will be automatically submitted immediately.)

Session Description
Each session at Klevay is a personalized experience, designed to support your individual healing journey. You’ll be gently guided in a safe and supportive space—with Phone Sessions—until Zoom Sessions become available. Every session is rooted in compassion, presence, and your unique path toward balance and renewal.
Cancellation & Rescheduling Policy
Your time and healing journey are deeply valued. To honor the commitment we both make in each session, I kindly ask for at least 24 hours notice for any cancellations or rescheduling.
- Cancellations made with less than 24 hours’ notice may be subject to a 50% cancellation fee.
- Missed appointments without notice (no-shows) may be subject to the full session fee.
Exceptions will, of course, be made for emergencies or unforeseen circumstances. Thank you for your understanding and for honoring the time we set aside for your healing journey.
